SHOW/HIDE isn't working as expected in Input Table on Card Form

    Hello! As I check your form; I noticed that in your conditions; the correct calculation widgets weren't considered for changing visibility of the fields. This could be happening because all of your calculation widgets are named "Calculation" only, which is confusing.

    It should work correctly. Please check it and in case you come across any issues; let us know.

    If you find no issues with this form; then following are the changes that I made in the form:

    1. I modified calculation widget names as "Calculation for Q1", "Calculation for Q2", "Calculation for Q3", "Calculation for Q4", "Calculation for Q5".

    2. After this, the conditions needs to be re-configured to use the right calculation widget.

    This is it. There is no other problem with the form. Please give it a try. Let us know if you come across any issues.

    So the form you sent works perfectly. I replicated the changes but I'm
    still running into the same issue. When I select "bad' or "somewhat bad"
    for all five of the options on the input table, all of the follow up
    questions should show up but only the first two do and the rest don't.
    Have I made all the same changes you did?
    Also, if there a way where you can force the input table to only make one
    selection per row? right now you are allowed to select multiple.

    I checked your form and I see you've hidden those questions:

    I need them to be hidden until the condition is met on the input table...
    When I test the form, the condition is met on the input table and the
    question doesn't show up.
