I need to reset my password so I can log into my account

    I need to login to my account to change the credit card on file. I forgot the password. I have clicked on the link to change my password many times but no email has been sent to me to change the password (yes, I checked the spam folder). Please help me change the password so I can update the credit card info.

    Unfortunately there is no account with email address Josh@bergenvet.com and that is the reason why you are not receiving any password reset email.

    What is your JotForm account username or the email address you have used in your account profile? Alternatively you can also let us know any of your form id, form URL or web page URL where you have embedded your form and we will trace your account.
