I need four box's that need to align both horizontally and Vertically, All in the same row. I can't seem to do it, it's either not justified correctly or just plain out of whack. Is it possible? Can you help.

To align two or more fields in the same line, you would need to shrink the fields first so they take less space.
To shrink, right click on the field and then choose Shrink
You can then drag the fields to align and place them where you want to.

It may be a bit tedious but using the numbers in this screenshot as a reference, could you please indicate where you wish each field in your form to be positioned in relation to the other fields?

I have tried to use this method but the field won't "stick" on the same line - it pops over or under the line I want it on. Any clues?

My hunch is that the form's width needs to be increased but I would need to see the form to be sure. Please provide its URL (found by clicking the Preview button in the toolbar) or, if applicable, the URL of the page in which it is embedded.

If my assumptions are correct, then this is probably what you're having problems with:
Like what Wayne has mentioned, your form's width cannot accommodate both fields in the same line, even if both fields are shrunk. Thus, your form's width should be increased.
If increasing the width is not an option, you may opt for any of the following solutions:
Add a Middle Name on your Full Name fields then replace "Middle Name" sub-label with "Maiden Name". Click on the sub-label itself to change it.
Decrease the size of the second field, so that it fits in the same line with the other field.
Set the label's alignment to "Top"
