How would I be able to setup a vaccine and insurance form

Thank you for your interest in JotForm. You can easily create an order form in the JotForm. Please follow these steps,

1) On the My Forms page, click the Create Form button and click the Start From Scratch.

Alternatively, you can select the "Use Template" option and view the "Pharmacy Forms" that are created by JotForm.

2) Once you have selected the Start From Scratch option, you will be asked to select the Layout of your form. Select the desired layout and continue. You will be directed to the Form Builder when you select the layout.

3) On the Form Builder, click the Add Form Elements button to add form fields to your form.

Under the Basic tab, you can find basic form elements such as Full Name, Address, and so on.

Under the Payments tab, you can find Payment Processor Integrations. The payment processor integrations will be useful if you wish to collect payment via your Forms.

Finally, under that Widgets tab, you can find useful widgets.
