Please Help ASAP

I created a form but it’s disappeared from my App on the iPad. It was fine when I used it about an hour ago with my previous class. Yet, it didn’t work for the class right after. The form disabled itself and I couldn’t locate it in the App.

So, what’s wrong with it? I still need the form for another class. Is it because this is a free account, so the form is buggy? Kindly advise please, ASAP. I’d thought that form functions were fantastic to work with and start with, but facing this issue in class, is really a very undesirable and embarrassing situation.
