Contact Forms

Hi,

We want to revamp the contact forms on our page.

Some Key Features we need to have:

1. Dynamic Subject Line. For example, the contact form should have 5 different options that need to have different email subjects.
2. The incoming form must be from the form fillers email, so that way when we reply back it goes straight to the form-filler
3. Some fields must be mandatory
4. After the form is completed there should be some kind of "Thank you for completing this form, a representative will reach back out to you soon" message or popup
5. Conditional Text Fields
6. Link with Klaviyo so we can set up an autoresponder

You can set up multiple email notification templates. Please check this guide: setting-up-email-notifications

The incoming form must be from the form fillers email, so that way when we reply back it goes straight to the form-filler

You can set the Reply-to Email option to the user's email so you'll be able to reply back to the user's via email.

Some fields must be mandatory.

Yes, you can set the fields mandatory or not.

After the form is completed there should be some kind of "Thank you for completing this form, a representative will reach back out to you soon" message or popup

You can set up a custom Thank You page and display it after form submission.
