User can't submit the form again

hey i have a form made for a hockey registration i have a parent that has two kids that need to be registered but it wont let her do the second kid

As per checking, you have enabled Unique Submissions in your form and set it to Check Cookies Only.

With this, one browser can only be used for one submission.

You may disable this so users can submit your form multiple times.
