Today's Date Calendar Isn't Working

One of my first questions on the form is "Today's Date". There's a small calendar icon next to it, but nothing happens when you click on it. When I try to fill the form out on my website and I manually type in the date, it acts like the question hasn't been answered. Very frustrating.

One other thing is the box is not centered in the web page, which looks kind of crappy.

We would like to apologize for any inconvenience. I see that you have embedded your form on a Wordpress.org website. May I ask what embed codes did you use to embed your form, please?

If it is not the same as what is instructed in this guide, A-guide-to-embed-forms-for-WordPress-users, may I suggest that you try updating the codes you have used based on what the guide suggests for embedding forms on a WordPress.org website, please?

Plug-in in the back end of my site. I opened a text editor and applied the
embed code in the html section. It shows up in the editor, but when I
update it, it doesn't show up on the page.
