Missing Document

I have created a document, and people have successfully completed the survey.

The following day, the document had disappeared from the Form Builder section of my account and is not displayed with the other created forms.

The document can be accessed from a direct link, but I can not access it to edit or see statics.

Where can I find the document?

It appears your account is on JotForm Enterprise version. I have forwarded your ticket to the Enterprise support team.
