I’m having trouble resetting my password

    I am trying to sign up got a COVID vaccine and having difficulty with the birthdate entry in my iPad. I switched to my laptop and the password would no longer work. When I tried to reset the password, it keeps saying that the password reset code has expired. I have tried this numerous times with the same result.

    As checked, your email isn't associated with any Jotform account, so password reset won't work either.

    If so, please sign up for an account first either using a desktop/laptop or our mobile app then you should see the ASSIGNED FORM there as soon as you logged in. Here's an example:
