My account is suspended

    Hello Jotform,

    For some reason my jotform account has been suspended due to attempt for phishing or collecting personal information. I don't know what's going on here. can you help me troubleshoot it?

    1. The URL where your forms are used/embedded.
    2. Your website or proof of affiliation to a legitimate institution.
    3. Share a document that will prove that you are authorized to offer such services.
