My account is suspended

I was creating an Opt-In Form for my Team Training Website where I require them to enter an Access Code or Password. But since I added that option to my Form, it has since Blocked my account. I have since deleted that Form hoping to resolve the issue.

Please reinstate my account.

Upon checking, your account was suspended due to the form http://form.jotform.com/210586383199264.

Your form is still showing on Trash.

However, I do see that the form is currently in the trash folder. In order for us to reactivate your account, we will have to permanently purge the faulty form from the trash folder.

Please let us know if you agree with the terms so we can proceed.

I no longer see the form which was the cause of your account being suspended. I have reactivated your account and you should be able to log in to your account again. Please feel free to let us know if you need any further assistance.
