    The form itself is fine on desktop browsers and mobile devices. It looks like that the issue is related to your responsive wordpress theme, it is resizing the div wrapper that is holding the form, so that part of the form is not visible. If your theme comes with support, please ask them to check this issue.

    Otherwise, we can try to check this further, but this might take some time. Thank you.

    Hello Dave,

    I'm sorry for the trouble caused to you.

    Upon checking your webpage, I found that you have used script embed code. Please use the following iFrame code to embed your form in your webpage:

    Now, please inject the following custom css in your from:

    @media only screen and (max-device-width: 480px)

    { iframe,.form-all { height:457px !important; } }

    Here is a guide to help you to inject the custom css in your from: http://www.jotform.com/help/117-How-to-Inject-Custom-CSS-Codes

    In your wordpress admin page, please open your theme css file in the wordpress editor and copy the following custom css there:

    iframe#JotFormIFrame { height:457px !important; }

    Hope this will solve your problem.

    Do get back to us if you have any questions.

    Thank you!

    Hello Dave,

    I am not sure how the iFrame code got deleted from my previous answer. Please use the following iFrame code along with the custom css which I have given above in my previous answer:

    <iframe id="JotFormIFrame" onload="window.parent.scrollTo(0,0)" allowtransparency="true" src="//www.jotform.com/form/33226086028956" frameborder="0" style="width:100%; height:457px; border:none;" scrolling="no"></iframe>

    Do get back to us if this solves your problem.

    Thank you!

    After checking your form in a phone other than iPhone I was able to reproduce the issue:

    So, in order to let the submit button appears we need to increase the height size, so please use the next CSS code:

    Insert this CSS code to your form:

    @media only screen and (max-device-width: 480px)
    { iframe,.form-all { height:550px !important; } }

    And in your wordpress admin page, please open your theme css file in the wordpress editor and copy the following custom css there:

    iframe#JotFormIFrame { height:550px !important; }

    As you can see the I have increased the height to 550px, so it will help to show the Submit button.

    Now, regarding to the message field, please give us some time so that we can find the correct code.

    Thanks for your comprehension.

    You will need to adjust your website styles to fix this issue.

    Please check the following css file:

    http://lasantefitness.com/wp-content/themes/sahifa/style.css

    You have two rules that are limiting the iFrame height on different screen sizes.

    First:

    Second:

    For example, you can try to change these values to 500px, then test your form.

    .content iframe { max-height: 500px; }

    Thank you.
