Smart PDF Form: newly-added fields are not added to the imported PDF

I am using a template of a form for a vaccine consent form. I added our information to the form but I am trying to add a section about the release of liability we have on our other vaccine consent forms. I add it successfully to the form but it does not print on the pdf version of the form and now there are all of these extra blue squares all over the form.

I can see that the form is a SMART PDF FORM. So if you manually added a new field into the web form, it won't update into imported PDF. You would need to update or edit the original PDF and re-upload it to the form.
