How to create a PDF document that looks the same as the form?

I have include three screen shots to illustrate one of my problems.

in picture 1 is the form as it appears on my website.

in picture 2 is the form as it appears after downloading.

Notice the downloaded version has 2 pages AND the items are out of sequence.

QUESTIONS: Why don't I see that a second page is being created, when I am building the form.

This is not browser specific as I am getting the same results in Chrome, Firefox and Edge.

How are the items getting out of sequence and is there a way to build the form in XML or can I see the raw code to make changes that way?

pic 3 is the actual paper form I am trying to duplicate, but I do not get the same results when trying to build this form in your program. I have watched tutorials on YouTube, but I am not getting the same results, which lead me to building this watered down version.
