I cloned the form and found the reason for not sending autoresponder mail.

To edit your Autoresponder Settings, please follow the steps below.

1. Open Settings and click the Emails. Click Autoresponder Mail Edit Button.
2. Click Recipient Tab. Select the Email field to Recipient Email option and Save changes.
3. Your email field is not required. Users may not fill. You can change the email field required properties.
