I need help logging in to my account

    Jotforms locked me out of the account. I sent a reset email and did not receive a reply. I need help now.

    I tried getting reset password instructions and have not received anything in reply. This has been a day already and I need access to patient forms for work. Please advise.

    Can you please help me. I have been trying to get access to forms for patients. I keep getting a response that the issue has been fixed, and it has not been. Thank you
