Why are my forms disabled?

Hey Keandra,
Thank you for your submission! Unfortunately, after reviewing your account, I found that your "Credit Services Intake Form" is in violation of our Terms of Use, which indicate that JotForm cannot be used to collect sensitive information such as Social Security Numbers, or Login Credentials- https://www.jotform.com/terms/
In order to re-activate your account and forms, these inquiries will need to be removed from the "Credit Services Intake Form."
Am I authorized to remove this form, or at least the offending fields, so that I can re-activate your account?

I've removed the offending fields from the form. I've also reactivated your account so you should be able to access it now.
