Disabled Form: Phishing for Login Credentials

    Hey Keandra,

    Thank you for your submission! Unfortunately, after reviewing your account, I found that your "Credit Services Intake Form" is in violation of our Terms of Use, which indicate that Jotform cannot be used to collect sensitive information such as Social Security Numbers, or Login Credentials- https://www.jotform.com/terms/

    In order to re-activate your account and forms, these inquiries will need to be removed from the "Credit Services Intake Form."

    Am I authorized to remove this form, or at least the offending fields, so that I can re-activate your account?

    i have removed all fields that are not allowed. My clients have sent me payments and I have no received any payments. They have sent me screenshots of them sending payments. I have not received anything.
