Name not pulling into table even though column is checked

Hello!

The data fields from 2 of our forms (GI and ENT - I'm in healthcare) are not pulling into our table. The columns are checked to show for each form in the table set-up, so not sure why data is not flowing over.

Our other 15 forms seem to be functioning fine, but these 2 are not.
