Can I create forms that I add specific details to before sending out link to customer?

Hi,

I'm wondering if I can do the following with JotForms:

Let's say I have a questionnaire that relates to a specific person that I need to send out. Can I put in the details for this person prior to sending out to the party I need answers back from regarding this person?
