Convert a PDF form to an online form for our agency.

Hello. I am trying to make an online application for our agency. We have a physical copy of our application. if we send you a pdf can you try to make an exact application online. also we would like to add a signature in the back.

Kindly note that to properly upload your original PDF form to JotForm to populate it using normal form fields, you may import it by clicking the "Create Form" button on your "My Forms" page, then follow the steps below.

Afterwards, you will be redirected to upload your PDF form, then you may easily link its fields to a form you would create using Form Builder.
