Why my account has been suspended? I cannot access my forms and my patients.
please let me know how to fix this issue

Your account has been suspended due to collecting credit card information. Kindly noted that collecting sensitive information such as credit card information, login credential, etc are violating our Term of Use. If you would require to accept payment in your form, kindly use one of our payment integration.
We can reactivate your account if you agree to comply with our Term of Use and allow us to delete the offending fields. Would you comply with our Term of Use and allow us to delete the offending fields?

Thank you for your response.We are not asking for credit card info we
charge them on PayPal on calendy software once they book their
appointment.we just ask them to consent to do the covid test and we have
charged them $125 ,but no where on form we are asking for credit card info

Hi,
As per checking, the Credit Card Authorization form found on your account triggered the suspension. Please note that the collection of credit card details using regular fields is against our terms of use.
Due to our PCI certification, we are required to deny the collection of credit card details on regular forms.
However, we have other PCI-compliant ways to do this. All you need to do is choose any of the payment processors available on our platform in order to integrate them into your forms. You still won't be able to collect credit card details directly, though.
If you only want to collect your customer's payment details and charge them at a later date, you can take advantage of a "Payment Authorization" feature. Kindly follow this guide on How to Enable Payment Authorization for more related information.
In order to reactivate the account, we need your permission to delete the offending field.
