When receiving the form in email, not all the fields show up. When I view the form in my account it looks different than when I get it in the email, (email form is missing 2-3 fieldsPage URL:

I've fixed it. Can you please try it again? If this happens to your forms, you can simply recreate the form notification email. First step would be to delete the current settings: How to Delete Notifications and second step is to re-Create: Setting Up Email Notifications.
