I need to insert a signature box on my form

I was told to click the add to pdf button. I don't see that anywhere and I also don't have a widgets tab. I really need a response asap as i need to create these forms and get them out today.

To add the signature in the Form you may add our signature widget. Below is the list of the widgets that you can add:
