    Hello, how can I incorporate this template: COVID-19 Restaurant Online Ordering Template: into my existing form?

    I assume that you are referring to the Theme. The Theme of the COVID-19 Restaurant Online Ordering Template is Brick Wall, with a different background image.

    Is it possible for me to use this template to modify my existing form without affecting the conditional setting?

    Your form conditions won't be affected if you switch over to any theme.
