Please Help - Account Suspended

Please help. My account is suspended. The form is for a job application, so it does ask for social security number, is that why the account has been suspended? We've been using that a jotform for this for a few years now, so why suspend now? We built a more simplified form for the same thing this week to try it out. Is that what got the account flagged? If necessary, we could remove the question about the SSN, but I can't even log in to the account to do that. Please help! My boss it freaking out because now even the old job application isn't working!
