I am seeking a product which will provide access to a survey or evaluation form through a weblink which when completed will merge specific data to a separate document (such as a certificate) and then deliver the document in PDF format to the email provided in the survey or evaluation to the one completing the survey or evaluation.
i.e., a student would click on a link or access with a QR Code the evaluation following a class. Part of the evaluation will collect an email address, name of student, class title and possible other informational fields. When the evaluation is completed/submitted, will JotForm technology pull the information provided in the fields completed by the student, merge to a certificate and then email the certificate as a PDF document to the student?
How is the response information from the evaluation stored for review by the educator?

Hello Jen - Will you be using a different application to create the certificate? Actually, you can do it all using Jotform alone.
Once you built the online form, you can create a custom submission PDF and design it as a certificate via the PDF Editor. There we have several certificate templates you can use:
You can then attach it to an email that gets sent every time the form is submitted. Here's a guide on how-to-include-submissions-as-pdf-attachment-in-the-email-notifications.
Also, you can add an email notification with the educators as the recipients. In that way, they will be able to receive an email as well when the form is submitted.
