your account settings link takes me to a page with a login prompt that my jotform credentials do not work for. I need to cancel my account immediately and do not wish to be charged again. Please instruct me on how to do this. I am available at 847-558-9910 and email@example.com

I can see in our database that you have already initiated the request to downgrade your JotForm account. It will take effect on 2011-06-18.
