PDF files do not support images after multi level approval

Hi, I am making Incident report form where when manager sign the form, PDF file should go to reporter/ injured person and manager. But, when manager receives PDF there are some images missing. It was there when manager receive it from the reporter for the signature, but after he signs it’s missing. It’s saying only png, jpeg, bmp images are supported but I’m using png file. Can anyone you please help me?

Form owner is me but I want to use it within our company. Whenever there is any incident report, I want Manager (Head of Operations) to sign it after he receives it, and sends that form back to report submitter and himself for record keeping.

To clarify, was the image uploaded missing in the form before the manager receives the form for final approval, or is the issue just with the attached PDF not showing the uploaded image?
