I am using form(Created by Jotform) on my website. When user submit form then i am getting empty email. I am attaching the screenshot please look.

Unfortunately, your from structure won't capture information that will be submitted to your email address and form submissions page. You have it as another input boxes inside our Free Text HTML field. That settings won't work.
There's currently no way you can create a narrative form with us. I would suggest creating a form and add our basic fields like, text box, full name and address to capture information from your users/customers. You should be able to see these fields from the form builder:

I am getting this issue when i create email (Email->Alert Compose Email). So there is possible to use "Quick Tool" ?
I am not sure if I have understood your question completely. Do you want to add fields from "Quick Tools" to the email template of notification alert?
Unfortunately, It is not possible to do so. Currently you can only customize your email template look and feel by using the toolbar. You can also add your own HTML code by clicking on the "HTML" button from toolbar. All form fields which you have added in your form is accessible in your email template.
