Some form submissions have PDF attachment, others don't

I have two forms that I would like a PDF version to come across with the notification email. This will make it easier to download into patients file.

The first one is Client Registration. https://www.jotform.com/build/201318407321140

Most times this does have a PDF, but everyone once in awhile does not and I can't see in the settings why.

The second one is Patient History. https://www.jotform.com/build/201412559638053

This one has never sent a PDF, even though the settings are identical to the one above.

On the Patient History, there is an option to upload pictures. The pictures have a link in the notification email, however the notification email link to these pictures take you to a 404 error.

    Please help

    Hello,

I have cloned your forms and tried to replicate the same issue on my end. Unfortunately, I could not do so. The PDF Attachments are working in both of the forms.

Here is a screencast of the first form,

1615441279 6049ad7fa38ec sc8 Screenshot 21 Please note that the attachment size is limited to 5MB. Currently, the attachment file size is dictated by the Email system, so you cannot change the max limit allowed within your Form Builder. Therefore, if the attachment size is bigger than 5MB, they are not coming with the email.

I have provided an answer to your second question in this thread,

Cannot View the Uploaded Images on the Notification Email

If you need further assistance, please let us know.

The PDF that is being sent is blank. It is not sending the completed form

Could you please try creating a new email notification template and attach the submission PDF there?

Remove the current email notification, then make a test submission. See if the PDFs now have data in them.

I have checked the PDFs on your form's PDF Editor and did not encounter a PDF with blank submissions in it.
