File upload field is not working in form.

My JotForm like hasn’t been working for clients, they can’t upload photos and aren’t able to fill out the survey.

I am sorry for the trouble caused to you. I did test your form, and I am able to replicate the issue. The file upload field was not responding. I am not sure what caused this issue but when I cleared your form cache, that seems to have fixed the issue.
