Limit access to the form

    I have a form for covid vaccinations but its being spread to people that I have not vaccinated and I dont know how to stop it from happening

    Hi there,

    Are you referring to your https://www.jotform.com/form/210584674824159 form?

    If you have the data or list of those who have an appointment, you can save it in Excel. Include a unique identifier for each record that would act as an access code, like a patient ID, or make up one for each record as long as it's unique and the patient knows about it.

    Use the Spreadsheet to form widget to upload your spreadsheet to your form. Label it with "Enter your unique code" or something similar. If the user entered the correct code, you can have the option to auto-populate the fields with the user's info using the following guide "How to Use the Spreadsheet to Form Widget".

    Set the widget to "required" so the users won't be able to send the form if they entered an incorrect code.
