Is this the only file you cannot download from your form submissions page? I can forward this to our developers if required. Otherwise, I would suggest contacting the user and ask to re-upload the file again and tell that the first attempt was not successful.
May we also know the website URL to where the form is currently placed? This way, we can check the page for a possible reason why the file has came but cannot be downloaded. A possible reason is a script conflict, preventing the form upload script from properly working.
