How can eliminate another companies title although I copied their form?

I cloned your form and checked. You can edit your form title by following the steps;

1. Click the “Settings” tab at the top of the form builder page.
2. Click the “Form Settings” at the left sidebar.
3. Scroll down and click the “Show More Options” button.
   
   
4. Scroll down to the “Page Title” section and update it as you want.
