I have created forms, I would like to know some info please...

We would like to purchase a package that will allow us to utilize them to send to our patients. When we do this, will we see the signature come over once we purchase a package?

Can we a quote for a month of the best package that you have?

Thank you for your inquiry regarding our package plans.

I see that you already have your clinic's logo with one of your forms in your account.

Once you purchase a paid membership package, you will have to add your refer the steps for adding company logo to your forms with below steps;

1. To upload a file from your local storage, select Upload.
2. To use a file that you have previously used, click on My Images.
3. Or, simply provide the link of the file from the Enter Url.

Or simply add the logo in the form;

Also, to remove JotForm branding, please go to your My Account > Settings page, then uncheck JotForm Branding.

As for the membership plans, there are Bronze, Silver and Gold, as well as the Enterprise memberships.

You will have to refer following page - https://www.jotform.com/pricing/ - to see which monthly membership plans suit your needs depending on the usage of the forms through your clinic.

Also, you will have to use the HIPAA compliant forms through the https://www.jotform.com/hipaa/ to collect sensitive health information from patients.

Thank you for your response. One more question, once the form is signed and
submitted does it show the signature on our end so that we can upload it to
the necessary areas?
I assume once we have a paid membership this is when all of those features
will show?
