Form not updating?

    I added a new field to this form: The field is a short text field called "Where did you hear about screening event?" and it should show up at the very end of the form, but it's not updating. It displays in the editor, but not on the live form. I cleared my Jotform account cache from my Settings page and also cleared my browser cache, and it's still not updating.

    Can you fix this?

    I checked your form and it seems it is updated correctly in the Share Link. It seems you are using Custom URL for your form, so please kindly try to re-apply Custom URL to see if that would fix the issue?
