Form fields not reflected in submitted forms

I am seeing incorrect fields show up in form responses.

Here is my website with contact form:

And here is what the result looks like... as you can see the fields are not the same. What is going on?

Hello,

Please reintegrate the email into the form - i.e. delete the existing email on the form and add a new one.

This will automatically update the email content with the updated fields from the form.
