What happens to the forms when you no longer subscribe to the service

    Hello,

I'm curious where the info in the forms is held (cloud server?) and how to access it. Also, if I decide to discontinue your service, how do I retain access to the records of the jot forms?

Is it possible to disable the scheduling feature?

Can I attach the forms as a link to an automated email where they are all hosted somewhere?

If two different appointment types require two slightly overlapping but different sets of forms, do I need to attached each form singularly or can I add a link to a group of forms?

Do patients get a reminder that they haven't completed their forms by a certain time?

Is it possible to see if a patient has viewed the forms?

    Thank you,

    Stefanie

The submissions are stored on our server. You can view the submissions via the Tables page:

    https://www.jotform.com/help/269-how-to-view-form-submissions

You may take a backup of your data if you decide to discontinue it.

May we know which scheduling feature are you referring to?

Yes, you may get the link to the form and attach the link to the emails:

    https://www.jotform.com/help/401-where-to-find-my-form-url

You may assign the forms to the user so that you can track the submissions, send a reminder emails:

    https://www.jotform.com/features/assign-forms/

Let us know if you have further questions.

    Thanks.
