I am sorry for the trouble caused to you. I did check your form and did not find any issue with that. I was able to submit your form successfully. Please check the screenshot below:

It seems you have embedded your form in your webpage. Please share the web page URL where you have embedded your form, and we will take a look.

We will wait for your response.

Hi, The web page URL is https://portal.acem.org.au/get-involved/demt-survey However this can only be accessed by our member with login details. Please help to recover the responses since the respondent has indicated that they will not be doing the survey again. Many thanks!

Please note that if the form was not submitted successfully, the data will not be captured. Since some error message was displayed as used, that means the submission was not successful, and unfortunately it is not possible to recover the submissions. You need to ask the user to submit the form again.

I am unable to check your embedded form as it requires login credentials. Which embed code you have used to embed form in your webpage? I would suggest you to please embed your form with its iframe embed code. The following guide should help you how to grab the form's iframe embed code: https://www.jotform.com/help/148-getting-the-form-iframe-code

Since you have a long and multi-page form, I would also suggest you to please enable "Continue Later" setting in form which will allow users to save the input data in form to ensure that data is not lost. You may like to take a look at the following guide which should help you: https://www.jotform.com/help/227-how-to-enable-autofill-on-forms

    Hope this helps.
