Second COVID vaccine appt

    I saw in your FAQ that someone had asked about automatically setting up the appt for the second shot when they do the first one and that is something that cannot be done. I want to know if there is a way to set a "stop" to the appointment if the person is not available at a set date and to have this change daily. For example:

    I certify that I am available (28 days from current date) for my second shot and will attend for my follow up appointment.


    I would like the patients to be aware of the two dates before proceeding.

    Yes, you can use conditional logic to disable the submit button or the appointment field if the user answer no to this question " I certify that I am available (28 days from current date) for my second shot and will attend for my follow up appointment.".

    Hi Jimmy_D. Thank you for your response. I am using the appointment feature with our COVID vaccine form. I would want to use this before/as they book the appointment. Is there a way to integrate it with the appointment calendar?

    Also, in the statement: " I certify that I am available (28 days from current date) for my second shot and will attend for my follow up appointment.", is there a way that I can populate the date they are selecting for the appointment?

    Do you mean to integrate the appointment field into your calendar? We do have a direct integration with Google Calendar. This helps you easily and seamlessly connect your form to a Google Calendar. Here's a guide:
