Form worked before, I just added a couple of new things and now it errors out - http://www.jotform.com/Urge/wholesale_order_formNot Found
The requested URL "Urge/wholesale_order_form" was not found on this server.
Can't fingure out why? any help would be great. As I sent the link to all my customers and am going to look bad if it's messed up.

I can confirm the existence of this issue. This seems to be a bug that was left unnoticed by our developers. With that in mind, I have submitted a report ticket to have this problem taken care of.
In the meantime, you can use this url instead

Yes, that's without the underscores.

Ok thanks. Just so you know I didn't put the underscores in, it happened automatically when I entered the name.

Yes I do realize that the link with the underscores were provided by the Embed Form wizard when you opted to use a custom URL.
Like I said I have reported this bug to our higher ups. They should be looking at it as soon as they become available.

Hi Urge and Neil
After running a couple of tests on one of my own forms, here's what I found:
As was stated before, if (for example) photo form (with a space) is typed into the "Custom URL" box, the wizard creates an invalid link ending in photo_form (with an underscore) but if I type photo_form, the wizard produces the very same link but for some reason this time it's valid! Strange indeed.
Anyway, I'm sure the developers will get to the bottom of this.

Fixed. You did not have underscores in the URL. You had spaces. I changed the URL to this:
