You can use the date calculation to calculate the second appointment date 28 days after the first appointment. Please refer to the steps I have outlined below.

1. You need to create another date picker field that holds the date of the second dose.
   
   
   
2. Add this date calculation to conditionally set the second dose's date 28 days after the first dose.
   
   
   
3. You can then add a paragraph text showing the statement, "I certify that I am available {dateOfSecondDose} for my second shot and will attend for my follow-up appointment."

Hi. I have set up our consent form with an appointment element. What I am trying to do is once the person selects a given time and date, then it automatically tells them what would be the date of their second dose and then confirm that they will be available on that date for their second shot.

I added the date picker as the example above but not sure what to enter in the condition segment so that the date of the second dose is calculated off of the date they choose for the appointment.

Now I am trying out the next step you had suggested: A paragraph statement "I certify that I am available {dateOfSecondDose} for my second shot and will attend for my follow-up appointment." However the date is not populating in the section {dateOfSecondDose}.

I could not find this paragraph field on any of your forms.

Have you removed it from your form?

It is the second paragraph in the input table property titled : Consent (check each box below...)

The section where I want the date of the second dose to be inserted is marked by {}.

Unfortunately, you can't get the value of the date picker field to the Input Table.

You can alternatively use the Multiple Choice field to display the paragraph.
