My patients data sheet form is missing!

Are you using the JotForm Health app to view your account?
I checked your account from our back end and can see the Patient Data sheet form, but only from the back end.
Since your account is HIPAA enabled, we are unable to log in and check inside your account.
Please log into your account using a web browser and check if the Patients Data sheet form is lying in the Trash folder or the Archive folder. You can see these folders on your My forms page:
After that, if using the JotForm Health app, please update the app to the latest version, log in and check.

why cant i edit the submission?

Were you able to access the Patient Datasheet form?
Are you editing the submission from the JotForm Health app or from the web browser in your account?
Please log in to your JotForm account using an incognito web browser window and check if you can edit the submission: https://www.jotform.com/help/325-how-to-edit-submissions-made-on-your-forms?
