Transferring form responses to a new form

Hello!

I work at a mental health care agency. I'd like to create an intake form for new clients. I would then like to create a new form with the information submitted in the intake form, minus the client's name and contact information, and share it with the therapists at our agency. This new form would say "Here is a new potential client - are you interested in meeting with them?"

Is this possible?

Hi, thank you for contacting support.

You can try to download your submissions as an Excel or CSV file, and then upload them using the Import App.

You can then delete that name and contact fields from the downloaded Excel/CSV before uploading the submissions using the import app.

This is the only option available for uploading submissions to other forms.
