can i create a grid/table where each box has a unique option to click a checkbox ?

Asked by schleichee on December 03, 2013 at 12:43 PM

I need to get employees to submit their class schedules/work availability online.

I have a table that shows all the class periods for Monday through Friday.  Essentially each box needs to have it's own text to identify it.  [see the screen shot].

I tried to make a grid, but since Tue/Thur have different time frames than Mon/Wed/Fri it made it difficult to have rows mixed that didn't have valid columns to match   [http://jotform.us/form/32335582492154]

I want to be able to replicate this in JotForm so that they can submit this to me and i can have all the schedule information as well as supplement info [name, special requests etc]

If this isn't possible - is there a way to link to a Word Document [like the attached screen shot file] that could be filled out and then saved to be submitted with the rest of the form?

    Thanks for any help.

I think you could embed a Google Calendar to accomplish your sceneario within your jotform using our Text tool which can be found under Form Tools

You need to share your google calendar (see this article for more details). Then simply drag and drop the Text fieldt, click on the HTML button at the toolbar and paste the Google Calendar code as shown below:
