Smart Pdf Forms not working

    Hello,

    I am trying to upload our pdf forms to the smart pdf forms and it is not working. I have tried multiple times and nothing is uploading. It just spins and spins for about 5 minutes and then when I go to look at "My forms" it has a list of forms there, but they are all empty. Does this work at all?

    Also, is there a way to embed the forms in our website
    , or
    are HIPPA forms only available via a separate link?

    We are trying out this service in place of another service we are using for our therapy clinic, and need for our clients to be able to fill out these forms and sign them online, without having to download, print and scan.

    I see you have a number of integrations, like Adobe Sign for example.

    Do we need to integrate with Adobe Sign to be able to have the clients sign the forms?

    I am mostly concerned with uploading our pdf's into smart pdf's. I have tried at least 6 times with various forms, short and long forms, and nothing has worked.
    Please help! One of the reasons we bought this service was this integration, we do not want to have to re-create our forms, as some of them are 5 pages long.

    Hi,

    Due to the HIPAA regulation we have, I can't login to your account nor locate the PDF file you would like to upload to SMART PDF. (I did not see the PDF file attached to the form with our cloned version)

    Since I cannot login to your account, I would like to ask you if you have followed the steps shown in guidelines such as https://www.jotform.com/help/873-how-to-update-the-pdf-through-smart-pdf-forms or https://www.jotform.com/help/548-how-can-i-import-my-own-pdf-with-smart-pdf-forms?

    If you have referenced them before and followed such steps but without any success, please share the corresponding copy or copies of the PDF for the specific forms, and we will try to see if we can re-upload them in our testing account.

    You can upload the PDFs to a cloud drive and share with us the link / links here that we can download.

    In the meantime, I have cleared your form caches. You might want to upload one more time before checking with the steps I have suggested.
