My form has a lot of Check Box items. I want people to be able to check each item that applies. When I preview the form here everything looks okay.
When I put it on my site (source code, embed, iFrame) three checkboxes appear for every item (Option 1, Option 2, Option 3). Let me know how I can correct this.

Please take a look at this sample form. Is that what you're trying to achieve?
In your form, you have created a checkbox field for every option, and that is not how it is done. The text Option 1, Option 2, and Option 3 can be replaced with the items that you want to put in your checkbox field.
Just click on the checkbox field, click on the Options button and then type in the items separated by a carriage return (Enter).
Apply the same for your other checkbox sections (Cooling, Heat, Communication, Laundry, etc.).

Thanks for the help. When I came back to the site to read these comments I see the that the preview should all three check boxes - not just the one I had been seeing before and that I had been trying to get. I changed the checkbox style to match your suggestion and it's working fine. Thanks again
