COVID appointment : Need to create an account?

    I received a COVID appointment prompt to sign up for an account. I did so but then did not see any forms so I canceled the account. It now will not let me re-create the account. Can someone assist me so that I can confirm the COVID appointment and get what is necessary to attend the appointment?

    It seems that you were sent a link from your local or private organization for administering and offering Corona vaccination.

    As you might be aware, we are an online form builder company, and I believe that you were contacted by someone / an organization that used our online form services.

    I have come across your email and I think you should contact the admin in charge of handling all the vaccination related tasks with cases such as yours;
